A physician can file his or her curriculum vitae (CV) with the Department of Workers’ Claims and request a Physician’s Medical Index Number. Once that number is assigned, an attorney can request the number and use that number as a reference on his/her pleadings, instead of attaching that physician’s CV to the pleading being filed. If the Administrative Law Judge or his/her staff wish to view a physician’s qualifications, they can bring up the Medical index number listed on the document in the Department’s Sharepoint database and view the CV.
