Reporting an Accident, Amputation, Loss of Eye, or Fatality

​Fat​​ality as a result of a work-related incident, including death resulting from a heart attack

  • Reporting Time: Within 8 hours
  • Report To: Division of OSH Compliance at (502) 564-3070 or to OSHA after hours at (800) 321-6742
  • Time Limitation: All fatalities must be reported; there is no time limitation

Ho​​spitalization of an employee including hospitalization resulting from a heart attack

  • Reporting Time: Within 72 hours
  • Report To: Division of OSH Compliance at (502) 564-3070 or to OSHA after hours at (800) 321-6742
  • Time Limitation: All formal admissions to the in-patient service of a hospital or clinic for care or treatment must be reported; there is no time limitation

Amp​​utation

    Reporting Time: Within 72 hours
  • Report To: ​Division of OSH Compliance at (502) 564-3070 or to OSHA after hours at (800) 321-6742
  • Time Limitation: All amputations must be reported; there is no time limitation

Loss of​​ eye

  • Reporting Time: Within 72 hours
  • Report To: Division of OSH Compliance at (502) 564-3070 or to OSHA after hours at (800) 321-6742
  • Time Limitation: All loss of eye injuries must be reported; there is no time limitation

No​tes
* Reporting time is the time from which an employer, employer’s agent, or another employee first becomes aware of the fatality, incident, amputation, or loss of eye.

* If the Frankfort office is closed and you cannot talk to a person, then you must call the OSHA toll-free number at 800-321-6742 and report ​​​the incident. Messages or faxes are not accepted to meet this reporting requirement.
